All new patients must have a valid credit card  at the time of booking an appointment

 

For medication changes and refill, please schedule an appointment, Do not have your local pharmacy request refill as we do not respond to these requests.

 

Benzodiazepines are controlled substances and can  have potential dangerous side effects. habit-forming. We can work with you in fiding alternative medications.

 

This practice does not accept patients that are on long term use of benzodiazepine

 

We do not prescribe Opioids or suboxone.
